The HTML5 Viewer is a client-side widget and must be added in the client part, and the Reporting REST service in the server part. For more details, please check the HTML5 Report Viewer overview.
The existing item template can only help you generate automatically the required HTML, but you will have to arrange the files based on the solution structure of your LightSwitch project (LightSwitch is not officially supported).
